Requesting a Copy of Your Records

A copy of your medical records may be obtained by downloading and printing the form below and bringing it to your doctor's office with a copy of your ID, bringing it to Kent Hospital's Health Information Management (HIM), medical records office, which is on the first floor of the main hospital or by mail. 

To request your records by mail, send the completed form with a copy of your ID to:

Birth Certificates

There are two ways to request a copy of a birth certificate: by mail or in person. Please be advised that in most cases, you will receive your records more quickly from the city or town locations than from the State Office.

By Mail:

Birth certificates can be obtained by writing to the Office of Vital Records, Room 101, Cannon Building, 3 Capitol Hill, Providence, RI 02908-5097 or to any city or town hall in Rhode Island. If you send for a birth certificate through the mail, you must include a photo copy of your driver's license.

In-Person:

Requests may be made in person at the state office address listed above or to any city or town hall in Rhode Island.
